Sick/Fit Note Certificates

"Off sick" for less than 7 days

If you have a medical cause of absence from work for 7 days or less, you do not require a Fit Note (MED3 or "sick certificate"). You can complete a Self-Certification form yourself. If your employer insists on a medical note or assessment, this should be provided by your employer's occupational health department/contractor. In exceptional circumstances we may be able to provide a letter. This typically involves a charge of £10.

Fit Note (MED3) for more than 7 days

If you have been ill for more than 7 days you will need a MED3 (Fit Note) certificate. These certificates are called ‘Fitness to Work’ Certificates.

For any condition resulting in absence from work for more than 7 days, you will require an initial medical assessment. Please contact reception to arrange an appointment. Your appointment may be with a GP, registrar, pharmacist, nurse practitioner or physiotherapist, depending on the nature of your problem. 

Additional Fit Notes / extensions

If you need subsequent Fit Notes (such as an extention) about the same problem, you may not need to see someone directly for this. Please contact reception for additional Fit Notes.
